Author Patrick Lencioni addresses this problem at the root level in his book The Five Dysfunctions of a Team. In it, he outlines how projects get off on the wrong foot from the beginning by not focusing on the development of a healthy team environment. These reasons are internal, and they imply all the responsibility in on the team, not external conditions. The strengths and weaknesses of teams can be very well handled, while threats from outside do not measure up to the opportunity, even when it is there in abundance.
